The encoded protein is a myosin IIA heavy chain that contains an IQ domain and a myosin head-like domain which is involved in several important functions, including cytokinesis, cell motility and maintenance of cell shape. Defects in this gene have been associated with non-syndromic sensorineural deafness autosomal dominant type 17, Epstein syndrome, Alport syndrome with macrothrombocytopenia, Sebastian syndrome, Fechtner syndrome and macrothrombocytopenia with progressive sensorineural deafness. This sequence change replaces serine, which is neutral and polar, with asparagine, which is neutral and polar, at codon 1519 of the MYH9 protein (p.Ser1519Asn). This variant is present in population databases (rs138193963, gnomAD 0.06%), and has an allele count higher than expected for a pathogenic variant. This variant has not been reported in the literature in individuals affected with MYH9-related conditions. ClinVar contains an entry for this variant (Variation ID: 164431). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.Ser1519Asn variant in MYH9 has been previously identified by our laboratory in two siblings with a suspected dominant family history of hearing loss, but has also been identified in 0.05% (14/24960) of African chromosomes by gnomAD (http://gnomad.broadinstitute.org). Computational prediction tools and conservation analysis suggest that this variant may not impact the protein, though this information is not predictive enough to rule out pathogenicity. In summary, while these data suggest that the p.Ser1519Asn variant is more likely to be benign, its clinical significance remains uncertain due to the presence of conflicting data.
