MYBPC3 is expressed exclusively in heart muscle and is a key regulator of cardiac contraction. Mutations in this gene are a frequent cause of familial hypertrophic cardiomyopathy. This variant is observed in 3/17,096 alleles from individuals of East Asian ancestry in large population cohorts (Lek et al., 2016). The C719R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. However, in-silico analyses, including protein predictors and evolutionary conservation, support that this variant does not alter protein structure/function.Therefore, based on the currently available information, it is unclear whether this variant is pathogenic or benign. -

This missense change has been observed in individual(s) with sudden death with suspected hypertrophic cardiomyopathy (PMID: 29219260). ClinVar contains an entry for this variant (Variation ID: 546308). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). Studies have shown that this missense change does not affect mRNA splicing (PMID: 34097875).
