Mutations in this gene are associated with Wolfram syndrome, also called DIDMOAD (Diabetes Insipidus, Diabetes Mellitus, Optic Atrophy, and Deafness), an autosomal recessive disorder. The disease affects the brain and central nervous system. Mutations in this gene can also cause autosomal dominant deafness 6 (DFNA6), also known as DFNA14 or DFNA38. The p.Ala433Thr variant in WFS1 has not been previously reported in individuals with hearing loss or Wolfram syndrome. This variant has been identified in 8/66 730 European chromosomes by the Exome Aggregation Consortium (ExAC, http://exac. broadinstitute.org; dbSNP rs138771366); however, its frequency is not high enoug h to rule out a pathogenic role. Computational prediction tools and conservation analyses suggest that the p.Ala433Thr variant may not impact the protein, thoug h this information is not predictive enough to rule out pathogenicity. In summar y, the clinical significance of the p.Ala433Thr variant is uncertain. -

This variant is present in population databases (rs138771366, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with WFS1-related conditions. ClinVar contains an entry for this variant (Variation ID: 166587). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt WFS1 protein function with a negative predictive value of 95%.
