The BRCA2 c.5406A>C; p.Gln1802His variant (rs139302211) is reported in the literature in individuals affected with breast or prostate cancer (Huang 2018, Lu 2015). This variant is also reported in ClinVar (Variation ID: 182217), but is only observed on two alleles in the Genome Aggregation Database, indicating it is not a common polymorphism. Computational analyses are uncertain whether this variant is neutral or deleterious (REVEL: 0.168), and this variant is not within a known clinically important functional domain (Dines 2020). However, given the lack of clinical and functional data, the significance of this variant is uncertain at this time. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in at least two individuals affected with breast cancer and in one unaffected individual (PMID: 26689913, 29625052, 33471991; Leiden Open Variation Database DB-ID BRCA2_008335). This variant has been identified in 2/251128 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.Q1802H variant (also known as c.5406A>C), located in coding exon 10 of the BRCA2 gene, results from an A to C substitution at nucleotide position 5406. The glutamine at codon 1802 is replaced by histidine, an amino acid with highly similar properties. This variant has been identified in two individuals from a cohort of 4034 cancer cases from The Cancer Genome Atlas (Lu C et al. Nat Commun. 2015 Dec 22;6:10086). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -

These report(s) do not provide unequivocal conclusions about association of the variant with Prostate Cancer. This variant is reported in 0.0062% of alleles in individuals of African descent in gnomAD and is interpreted as uncertain by multiple laboratories in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/182217/). This variant occurs within a region of the BRCA2 gene that is predicted to be tolerant to missense variation (Table 2, Dines et al. 2020. PubMed ID: 31911673). Although we suspect that this variant may be benign, the clinical significance of this variant is uncertain at this time due to the absence of conclusive functional and genetic evidence. -
