Variant classified as Uncertain Significance - Favor Benign. The p.Leu16074Gln v ariant in TTN has been identified by our laboratory in 5 Caucasian individuals w ith different cardiomyopathies (1 adult with DCM, 1 adult with DCM and skeletal myopathy, 1 adult with LVNC, 1 teenager with DCM and syndromic features, and 1 i nfant with HCM). This variant has been identified in 50/66574 European chromosom es by the Exome Aggregation Consortium (ExAC, http://exac.broadinstitute.org; db SNP rs140714512). Computational prediction tools and conservation analysis sugg est that the p.Leu16074Gln variant may not impact the protein, though this infor mation is not predictive enough to rule out pathogenicity. In summary, while the clinical significance of the p.Leu16074Gln variant is uncertain, its frequency and lack of conservation suggests that it is more likely to be benign. -
Variant summary: TTN c.48221T>A (p.Leu16074Gln) results in a non-conservative amino acid change located in the A-band domain of the encoded protein sequence. Two of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00051 in 248080 control chromosomes in the gnomAD database, including 1 homozygotes. This frequency is not significantly higher than estimated for a pathogenic variant in TTN causing Cardiomyopathy (0.00051 vs 0.00063), allowing no conclusion about variant significance. c.48221T>A has been reported in the literature in individuals affected with sudden unexplained death and dilated cardiomyopathy, without strong evidence for causality (Campuzano_2015, Burstein_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 26516846, 32746448). ClinVar contains an entry for this variant (Variation ID: 177978). Based on the evidence outlined above, the variant was classified as uncertain significance. -
Autosomal recessive limb-girdle muscular dystrophy type 2J Uncertain:1
This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -
