Variant summary: CPT2 c.912G>T (p.Gln304His) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-05 in 251412 control chromosomes (gnomAD). This frequency is not significantly higher than estimated for a pathogenic variant in CPT2 causing Carnitine Palmitoyltransferase II Deficiency (8e-05 vs 0.0016), allowing no conclusion about variant significance. c.912G>T has been reported in the literature in individuals affected with hyperCKemia, myalgia, and exercise intolerance without strong evidence of causality (Invernizzi_2023). This report does not provide unequivocal conclusions about association of the variant with Carnitine Palmitoyltransferase II Deficiency. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
