Variant summary
Our verdict is Likely benign. Variant got -1 ACMG points: 0P and 1B. BP4
The NM_000179.3(MSH6):c.3847A>G(p.Ile1283Val) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000479 in 1,461,810 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

Hereditary cancer-predisposing syndrome Uncertain:3
This missense variant replaces isoleucine with valine at codon 1283 of the MSH6 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has been identified in 3/250882 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.I1283V variant (also known as c.3847A>G), located in coding exon 9 of the MSH6 gene, results from an A to G substitution at nucleotide position 3847. The isoleucine at codon 1283 is replaced by valine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
BP4 c.3847A>G, located in exon 9 of the MSH6 gene, is predicted to result in the substitution of isoleucine by valine at codon 3847, p.(Ile1283Val).This variant is found in 3/267738 alleles at a frequency of 0.001% in the gnomAD v2.1.1 database, non-cancer dataset. Computational tools for this variant suggests no significant impact on splicing and does not affect the protein function (MAPP+PolyPhen-2 prior probability for pathogenicity: 0.10) (BP4). To our knowledge, neither relevant clinical data nor well-established functional studies have been reported for this variant. This variant has been reported in the ClinVar database (3x uncertain significance, 1x likely benign), but it has not been reported in LOVD or classified by InSiGHT. Based on currently available information, the variant c.3847A>G should be considered an uncertain significance variant. -
