The p.E468K variant (also known as c.1402G>A), located in coding exon 9 of the RAD50 gene, results from a G to A substitution at nucleotide position 1402. The glutamic acid at codon 468 is replaced by lysine, an amino acid with similar properties. This variant was identified in 3/3,579 African males diagnosed with prostate cancer who underwent multi-gene panel testing of 19 DNA repair and cancer predisposition genes (Matejcic M et al. JCO Precis Oncol, 2020 Jan;4:32-43). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 468 of the RAD50 protein (p.Glu468Lys). This variant is present in population databases (rs145031602, gnomAD 0.05%). This variant has not been reported in the literature in individuals affected with RAD50-related conditions. ClinVar contains an entry for this variant (Variation ID: 141052). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt RAD50 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The RAD50 c.1402G>A (p.Glu468Lys) variant has been reported in the published literature in an individual affected with diverse cancers, including breast cancer, who also carried other genetic variants (PMID: 35534704 (2022)). This variant was also reported in an undescribed carrier in a study of prostate cancer (PMID: 32832836 (2020)). The frequency of this variant in the general population, 0.00056 (14/24826 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded conflicting predictions that this variant is benign or damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -
