Therefore, it has been classified as a Variant of Uncertain Significance. Studies have shown that this variant results in activation of a cryptic splice site and introduces a premature termination codon (PMID: 26740943). The resulting mRNA is expected to undergo nonsense-mediated decay. ClinVar contains an entry for this variant (Variation ID: 1734525). This variant has been observed in individual(s) with clinical features of neurofibromatosis type 1 and/or neurofibromatosis type 1 (PMID: 26740943; Invitae). This variant is not present in population databases (gnomAD no frequency). This sequence change affects codon 1250 of the NF1 mRNA. It is a 'silent' change, meaning that it does not change the encoded amino acid sequence of the NF1 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product. - |

This nucleotide substitution does not change the amino acid arginine at codon 1250. This variant was detected in a child under the age of two with several features suggestive of NF1. In addition, this variant was predicted at the mRNA level to have an impact on splicing, producing a transcript with an out of frame deletion (r.3709_3749del41) resulting in a reading frameshift and premature stop codon (Bianchessi D, et al. Mol Genet Genomic Med 2015; 3(6):513-25). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This nucleotide position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to create a new alternate splice acceptor site; however, direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
