A variant of uncertain significance has been identified in the CLN3 gene. The H404Y variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The H404Y variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In silico analysis predicts this variant is probably damaging to the protein structure/function. However, this substitution occurs at a position where amino acids with similar properties to Histidine are tolerated across species. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant. -
Neuronal ceroid lipofuscinosis Uncertain:1
This sequence change replaces histidine, which is basic and polar, with tyrosine, which is neutral and polar, at codon 404 of the CLN3 protein (p.His404Tyr). This variant is present in population databases (rs146610181, gnomAD 0.01%). This missense change has been observed in individual(s) with clinical features of neuronal ceroid lipofuscinosis (PMID: 32631363). ClinVar contains an entry for this variant (Variation ID: 421929).
