FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. The arginine at codon 74 is replaced by serine, an amino acid with dissimilar properties. This alteration has been identified in individuals with a personal and/or family history of paragangliomas or pheochromocytomas (PGL/PCC), including tumors with a loss of fumarate hydratase staining in immunohistochemistry experiments (Ambry internal data; Fuchs TL et al. Am J Surg Pathol 2023 Jan;47(1):25-36; Zavoshi S et al. Urology 2023 Feb.). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the supporting evidence, this alteration is interpreted as pathogenic in association with PGL/PCC, however its association with other features of hereditary leiomyomatosis and renal cell carcinoma syndrome is unclear. -
