Variant summary: ATP7B c.3889G>A (p.Val1297Ile) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.0027 in 279784 control chromosomes, predominantly at a frequency of 0.017 within the East Asian subpopulation in the gnomAD database, including 6 homozygotes. The observed variant frequency within East Asian control individuals in the gnomAD database is approximately 3.15 fold of the estimated maximal expected allele frequency for a pathogenic variant in ATP7B causing Wilson Disease phenotype (0.0054), strongly suggesting that the variant is a benign polymorphism found primarily in populations of East Asian origin. c.3889G>A has been reported as a "polymorphism" in the literature in individuals affected with Wilson Disease. Multiple studies reported not significantly different MAF of variant in diesease vs control cohort (Dong_2016 and Hua_2016) and listed variant as polymorphism. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar without evidence for independent evaluation (2 benign, 2 likely benign and 1 VUS). Based on the evidence outlined above, the variant was classified as benign. -
