Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_014140.4(SMARCAL1):c.2321C>A(p.Ser774*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,461,886 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. S774S) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Variant summary: SMARCAL1 c.2321C>A (p.Ser774X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant allele was found at a frequency of 8e-06 in 251442 control chromosomes (gnomAD). c.2321C>A has been observed in individuals affected with Schimke Immunoosseous Dysplasia (Boerkoel_2002). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 11799392). ClinVar contains an entry for this variant (Variation ID: 463148). Based on the evidence outlined above, the variant was classified as pathogenic. -
This sequence change creates a premature translational stop signal (p.Ser774*) in the SMARCAL1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in SMARCAL1 are known to be pathogenic (PMID: 11799392, 20301550). This variant is present in population databases (rs149425324, gnomAD 0.003%). This premature translational stop signal has been observed in individual(s) with clinical features of Schimke immunoosseous dysplasia (PMID: 11799392). ClinVar contains an entry for this variant (Variation ID: 463148). For these reasons, this variant has been classified as Pathogenic. -
