This variant is present in population databases (rs149651434, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with FH-related conditions. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt FH protein function with a positive predictive value of 95%. The p.F225S variant (also known as c.674T>C), located in coding exon 5 of the FH gene, results from a T to C substitution at nucleotide position 674. The phenylalanine at codon 225 is replaced by serine, an amino acid with highly dissimilar properties. This alteration was identified in 1/1358 non-cancer control individuals and in 1/57 cases, in a study looking at cancer predisposition mutations in patients with cutaneous melanoma and a history of at least two additional non-cutaneous melanoma primary cancers (Pritchard AL et al. PLoS One, 2018 Apr;13:e0194098). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
