This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. The p.D1080E variant (also known as c.3240C>A), located in coding exon 21 of the ATM gene, results from a C to A substitution at nucleotide position 3240. The aspartic acid at codon 1080 is replaced by glutamic acid, an amino acid with highly similar properties. This alteration has been reported in the germline of a male diagnosed with acute mega-karyoblastic leukemia and synchronous mediastinal germ cell tumor of common clonal origin at age 17 (Oshrine BR et al. Cancer Genet 2014 Apr; 207(4):153-9). This alteration has also been reported in a cohort of 488 patients with stages I to III breast cancer who were tested with a 25-gene panel test (Tung N et al. J. Clin. Oncol., 2016 May;34:1460-8). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 251264 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3240C>A has been reported in the literature as a VUS in an individual with breast cancer (example, Tung_2016), a study cohort of individuals undergoing WES for neurodevelopmental disorders without a cancer diagnosis (example, Kraemer_2019), a male with acute megakaryoblastic leukemia and mediastinal germ cell tumor (example, Oshrine_2014). These report(s) do not provide unequivocal conclusions about association of the variant with Breast Cancer/Ataxia Telangiectasia. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.

This variant is denoted ATM c.3240C>A at the cDNA level, p.Asp1080Glu (D1080E) at the protein level, and results in the change of an Aspartic Acid to a Glutamic Acid (GAC>GAA). This variant has been observed in at least one patient with breast cancer as well as in an individual with concurrent acute megakaryoblastic leukemia and a mediastinal germ cell tumor, and was absent from 400 control samples (Oshrine 2014, Tung 2016). ATM Asp1080Glu was observed at an allele frequency of 0.026% (4/15,300) in individuals of African ancestry in large population cohorts (Lek 2016). This variant is located within the region of interaction with beta-adaptin (Tavtigian 2009). In-silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. Based on currently available evidence, it is unclear whether ATM Asp1080Glu is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
