It is an enzyme that catalyzes the interconversion of citrate to isocitrate via cis-aconitate in the second step of the TCA cycle. This protein is encoded in the nucleus and functions in the mitochondrion. It was found to be one of the mitochondrial matrix proteins that are preferentially degraded by the serine protease 15(PRSS15), also known as Lon protease, after oxidative modification. This sequence change replaces proline, which is neutral and non-polar, with serine, which is neutral and polar, at codon 330 of the ACO2 protein (p.Pro330Ser). This variant is present in population databases (rs150129663, gnomAD 0.02%). This missense change has been observed in individual(s) with clinical features of autosomal recessive infantile cerebellar-retinal degeneration (PMID: 37460232). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 520988). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt ACO2 protein function with a negative predictive value of 80%. The c.988C>T (p.P330S) alteration is located in exon 8 (coding exon 8) of the ACO2 gene. This alteration results from a C to T substitution at nucleotide position 988, causing the proline (P) at amino acid position 330 to be replaced by a serine (S). Based on data from gnomAD, the T allele has an overall frequency of 0.002% (5/282722) total alleles studied. The highest observed frequency was 0.02% (5/24964) of African alleles. This variant has been identified in conjunction with another ACO2 variant in an individual with a personal and family history of global developmental delay, abnormal eye movements, and suspected seizure episodes, and with a personal history of hypotonia, maculopathy and retinal dystrophy, absent speech, and other clinical features consistent with ACO2-related infantile cerebellar-retinal degeneration (Lail, 2023). This amino acid position is not well conserved in available vertebrate species. This alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, this alteration is classified as likely pathogenic. -

Variant summary: ACO2 c.988C>T (p.Pro330Ser) results in a non-conservative amino acid change located in the Aconitase iron-sulfur domain (IPR036008) of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251352 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.988C>T has been reported in the literature in at least one compound heterozygous individual affected with ACO2-Related Disorders (Lail_2023). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
