Genes affected
FHL1 (HGNC:3702): (four and a half LIM domains 1) This gene encodes a member of the four-and-a-half-LIM-only protein family. Family members contain two highly conserved, tandemly arranged, zinc finger domains with four highly conserved cysteines binding a zinc atom in each zinc finger. Expression of these family members occurs in a cell- and tissue-specific mode and these proteins are involved in many cellular processes. Mutations in this gene have been found in patients with Emery-Dreifuss muscular dystrophy. Multiple alternately spliced transcript variants which encode different protein isoforms have been described.[provided by RefSeq, Nov 2009]

Likely benign, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpJun 01, 2022Variant summary: FHL1 c.283C>T (p.Arg95Trp) results in a non-conservative amino acid change located in the Zinc finger, LIM-type domain (IPR001781) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.00068 in 182189 control chromosomes with 1 homozygote and 42 hemizygotes (gnomAD). The variant occurred predominantly at a frequency of 0.013 within the Ashkenazi Jewish subpopulation in the gnomAD database, including 35 hemizygotes. The observed variant frequency within Ashkenazi Jewish control individuals in the gnomAD database is approximately 34 fold of the estimated maximal expected allele frequency for a pathogenic variant in FHL1 causing Emery-Dreifuss Muscular Dystrophy phenotype (0.00039), strongly suggesting that the variant is a benign polymorphism found primarily in populations of Ashkenazi Jewish origin. c.283C>T has been reported in the literature in individuals affected with peripheral neuropathy and muscle weakness (Selcen_ 2011), pediatric cardiomyopathy (Kuhnisch_2019) and hypoparathyroidism (Pillar_2017). These reports do not provide unequivocal conclusions about association of the variant with Emery-Dreifuss Muscular Dystrophy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three ClinVar submitters have assessed the variant since 2014: two classified the variant as likely benign and one as benign. Based on the evidence outlined above, the variant was classified as likely benign. -
