This sequence change replaces threonine, which is neutral and polar, with alanine, which is neutral and non-polar, at codon 65 of the MPZ protein (p.Thr65Ala). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Charcot-Marie-Tooth disease (PMID: 15036333, 30677751). ClinVar contains an entry for this variant (Variation ID: 447727). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on MPZ protein function. Experimental studies have shown that this missense change affects MPZ function (PMID: 29687021). This variant disrupts the p.Thr65 amino acid residue in MPZ. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 20456450). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively.
