Variant summary

Our verdict is Pathogenic. Variant got 16 ACMG points: 16P and 0B. PM1PM2PM5PP3_ModeratePP5_Very_Strong

The NM_001875.5(CPS1):c.3980G>A(p.Cys1327Tyr) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. C1327R) has been classified as Likely pathogenic.

Genes affected
CPS1 (HGNC:2323): (carbamoyl-phosphate synthase 1) The mitochondrial enzyme encoded by this gene catalyzes synthesis of carbamoyl phosphate from ammonia and bicarbonate. This reaction is the first committed step of the urea cycle, which is important in the removal of excess urea from cells. The encoded protein may also represent a core mitochondrial nucleoid protein. Three transcript variants encoding different isoforms have been found for this gene. The shortest isoform may not be localized to the mitochondrion. Mutations in this gene have been associated with carbamoyl phosphate synthetase deficiency, susceptibility to persistent pulmonary hypertension, and susceptibility to venoocclusive disease after bone marrow transplantation.[provided by RefSeq, May 2010]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 16 ACMG points.

PM1
In a strand (size 6) in uniprot entity CPSM_HUMAN there are 7 pathogenic changes around while only 0 benign (100%) in NM_001875.5
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chrnull-null-null-null is described in UniProt as null.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.884
PP5
Variant 2-210663175-G-A is Pathogenic according to our data. Variant chr2-210663175-G-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 488489.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Likely pathogenic, criteria provided, single submitterclinical testing3billionJan 03, 2022Same nucleotide change resulting in same amino acid change has been previously reported to be associated with CPS1 related disorder (ClinVar ID: VCV000488489, PMID:28526534, PS1_P). A different missense change at the same codon has been reported to be associated with CPS1 related disorder (PMID:21120950, PM5_P). In silico tool predictions suggest damaging effect of the variant on gene or gene product (REVEL: 0.846, 3CNET: 0.876, PP3_P). It is not observed in the gnomAD v2.1.1 dataset (PM2_M). Therefore, this variant is classified as likely pathogenic according to the recommendation of ACMG/AMP guideline. -
