The p.G422D variant (also known as c.1265G>A), located in coding exon 12 of the MLH1 gene, results from a G to A substitution at nucleotide position 1265. The glycine at codon 422 is replaced by aspartic acid, an amino acid with similar properties. This amino acid position is conserved. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
PM2_Supporting, BP4 c.1265G>A, located in exon 12 of the MLH1 gene, is predicted to result in the substitution of Gly by Asp at codon 422, p.(Gly422Asp). It is not present in the population database gnomAD v2.1.1, non cancer dataset (PM2_supporting). Computational tools for this variant suggests no significant impact on splicing and does not affect the protein function (MAPP+PolyPhen-2 prior probability for pathogenicity: 0.015) (BP4). To our knowledge, neither individuals with Lynch syndrome-related conditions nor functional studies have been reported in the literature for this variant. There are no reports of pathogenic missense variants in the same codon. In addition, this variant has been reported in the ClinVar database (3x uncertain significance) but it has not been identified neither in LOVD nor InSiGHT databases. Based on currently available information, the variant c.1265G>A should be considered an uncertain significance variant. -

This variant has not been reported in individuals with MLH1-related conditions in the published literature. It also has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

This sequence change replaces glycine with aspartic acid at codon 422 of the MLH1 protein (p.Gly422Asp). The glycine residue is weakly conserved and there is a moderate physicochemical difference between glycine and aspartic acid. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with MLH1-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
