Genes affected
ARSB (HGNC:714): (arylsulfatase B) Arylsulfatase B encoded by this gene belongs to the sulfatase family. The arylsulfatase B homodimer hydrolyzes sulfate groups of N-Acetyl-D-galactosamine, chondriotin sulfate, and dermatan sulfate. The protein is targeted to the lysozyme. Mucopolysaccharidosis type VI is an autosomal recessive lysosomal storage disorder resulting from a deficiency of arylsulfatase B. Two alternatively spliced transcript variants encoding distinct isoforms have been found for this gene. [provided by RefSeq, Dec 2016]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 12 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.0156 CDS is truncated, and there are 0 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 5-78780421-AG-A is Pathogenic according to our data. Variant chr5-78780421-AG-A is described in ClinVar as [Pathogenic]. Clinvar id is 559724.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Experimental studies have shown that this frameshift affects ARSB function (PMID: 8116615). Algorithms developed to predict the effect of variants on protein structure and function are not available or were not evaluated for this variant. ClinVar contains an entry for this variant (Variation ID: 559724). This frameshift has been observed in individual(s) with mucopolysaccharidosis type VI (PMID: 8116615, 24677745). This variant is not present in population databases (gnomAD no frequency). This sequence change results in a frameshift in the ARSB gene (p.Thr526Metfs*48). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 8 amino acid(s) of the ARSB protein and extend the protein by 39 additional amino acid residues. For these reasons, this variant has been classified as Pathogenic. -
