Variant summary
Our verdict is Pathogenic. The variant received 12 ACMG points: 12P and 0B. PVS1PM2PP5_Moderate
The NM_000038.6(APC):c.2021T>A(p.Leu674*) variant causes a stop gained change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★). Synonymous variant affecting the same amino acid position (i.e. L674L) has been classified as Benign.

Familial adenomatous polyposis 1 Pathogenic:1
For these reasons, this variant has been classified as Pathogenic. Several different truncations downstream of this variant (p.Trp699*, p.Ser932*, p.Ala1050Glufs*6, p.Gln1062*) have been determined to be pathogenic (PMID: 10083733, 15771908, 20223039, 20685668, 23460355). This suggests that deletion of this region of the APC protein is causative of disease. This variant has not been reported in the literature in individuals with APC-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change results in a premature translational stop signal in the APC gene (p.Leu674*). While this is not anticipated to result in nonsense mediated decay, it is expected to delete the last 2,170 amino acids of the APC protein. -
