The p.Gly1039LysfsX8 deletion variant has been previously reported in the literature in 1 family with FAP (Cao 2006). This deletion is predicted to cause a frameshift, which alters the protein's amino acid sequence beginning at codon 1039 and leads to a premature stop codon 8 codons downstream. This alteration is predicted to cause a truncated or absent protein and loss of function. Loss of function variants are an established mechanism of disease for the APC gene and is the they type of variant that is expected to cause FAP. In summary, based on the above information, this variant is classified as pathogenic. -

The c.3114_3115delTG pathogenic mutation, located in coding exon 15 of the APC gene, results from a deletion of two nucleotides at nucleotide positions 3114 to 3115, causing a translational frameshift with a predicted alternate stop codon (p.G1039Kfs*8). This alteration occurs at the 3' terminus of theAPC gene and is not expected to trigger nonsense-mediated mRNA decay. However, premature stop codons are typically deleterious in nature, the impacted region is critical for protein function, and a significant portion of the protein is affected (Ambry internal data). This alteration was identified in a patient with familial adenomatous polyposis (FAP) (Cao X et al. Am J Gastroenterol, 2006 Dec;101:2810-7). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -
