Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_004752.4(GCM2):c.408C>A(p.Tyr136Ter) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Genes affected
GCM2 (HGNC:4198): (glial cells missing transcription factor 2) This gene is a homolog of the Drosophila glial cells missing gene, which is thought to act as a binary switch between neuronal and glial cell determination. The protein encoded by this gene contains a conserved N-terminal GCM motif that has DNA-binding activity. The protein is a transcription factor that acts as a master regulator of parathyroid development. It has been suggested that this transcription factor might mediate the effect of calcium on parathyroid hormone expression and secretion in parathyroid cells. Mutations in this gene are associated with hypoparathyroidism. [provided by RefSeq, Jul 2008]

Pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Feb 04, 2019 | The Y136X variant in the GCM2 gene has been reported previously as homozgyous in three siblings with severe congenital hypoparathyroidism (Doyle et a., 2012). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The Y136X variant is not observed in large population cohorts (Lek et al., 2016). We interpret Y136X as a pathogenic variant. - |
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jun 21, 2023 | For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 433164). This premature translational stop signal has been observed in individual(s) with congenital hypoparathyroidism (PMID: 23155703). It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Tyr136*) in the GCM2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in GCM2 are known to be pathogenic (PMID: 20190276, 23155703). - |
