The p.I340N variant (also known as c.1019T>A), located in coding exon 8 of the CFTR gene, results from a T to A substitution at nucleotide position 1019. The isoleucine at codon 340 is replaced by asparagine, an amino acid with dissimilar properties. In an assay testing CFTR function, this variant showed a functionally abnormal result (Han ST et al. JCI Insight, 2018 Jul;3:).This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

Variant summary: CFTR c.1019T>A (p.Ile340Asn) results in a non-conservative amino acid change located in the ABC transporter type 1, transmembrane domain (IPR011527) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251226 control chromosomes (gnomAD). To our knowledge, no occurrence of c.1019T>A in individuals affected with Cystic Fibrosis has been reported. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in 10%-<30% of normal activity (e.g. Enquist_2009, Han_2018). The following publications have been ascertained in the context of this evaluation (PMID: 19236881, 25735457, 25192979, 30046002). ClinVar contains an entry for this variant (Variation ID: 495884). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
