Genes affected
KCTD7 (HGNC:21957): (potassium channel tetramerization domain containing 7) This gene encodes a member of the potassium channel tetramerization domain-containing protein family. Family members are identified on a structural basis and contain an amino-terminal domain similar to the T1 domain present in the voltage-gated potassium channel. Mutations in this gene have been associated with progressive myoclonic epilepsy-3. Alternative splicing results in multiple transcript variants.[provided by RefSeq, Jan 2011]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.19425288 fraction of the gene. Cryptic splice site detected, with MaxEntScore 5.7, offset of -33, new splice context is: ccgGTactt. Cryptic site results in inframe change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 7-66633445-G-A is Pathogenic according to our data. Variant chr7-66633445-G-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 536951.Status of the report is criteria_provided_single_submitter, 1 stars.

Progressive myoclonic epilepsy type 3 Pathogenic:1
Likely pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpJan 24, 2018In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Donor and acceptor splice site variants typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in KCTD7 are known to be pathogenic (PMID: 22693283). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site, but this prediction has not been confirmed by published transcriptional studies. This variant has not been reported in the literature in individuals with KCTD7-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change affects a donor splice site in intron 2 of the KCTD7 gene. It is expected to disrupt RNA splicing and likely results in an absent or disrupted protein product. -
