Genes affected
TWNK (HGNC:1160): (twinkle mtDNA helicase) This gene encodes a hexameric DNA helicase which unwinds short stretches of double-stranded DNA in the 5' to 3' direction and, along with mitochondrial single-stranded DNA binding protein and mtDNA polymerase gamma, is thought to play a key role in mtDNA replication. The protein localizes to the mitochondrial matrix and mitochondrial nucleoids. Mutations in this gene cause infantile onset spinocerebellar ataxia (IOSCA) and progressive external ophthalmoplegia (PEO) and are also associated with several mitochondrial depletion syndromes. Alternative splicing results in multiple transcript variants encoding distinct isoforms.[provided by RefSeq, Aug 2009]

Based on the classification scheme VCGS_Germline_v1.3.4, this variant is classified as Pathogenic. Following criteria are met: 0103 - Dominant negative and loss of function are known mechanisms of disease in this gene and are associated with progressive external ophthalmoplegia with mitochondrial DNA deletions 3 (MIM#609286), mitochondrial DNA depletion syndrome 7 (hepatocerebral type) (MIM#271245) and Perrault syndrome 5 (MIM#616138) (PMID: 18971204). (I) 0108 - This gene is associated with both recessive and dominant disease. Mitochondrial DNA depletion syndrome 7 (hepatocerebral type) (MIM#271245) and Perrault syndrome 5 (MIM#616138) are inherited in a recessive manner, whereas progressive external ophthalmoplegia with mitochondrial DNA deletions 3 (MIM#609286) is a dominant condition (OMIM). (I) 0200 - Variant is predicted to result in a missense amino acid change from glutamine to histidine. (I) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (both v2 and v3). (SP) 0501 - Missense variant consistently predicted to be damaging by multiple in silico tools or highly conserved with a major amino acid change. (SP) 0600 - Variant is located between two Walker a/b motifs and within the helicase functional domain (PMID: 20479361). (I) 0705 - No comparable missense variants have previous evidence for pathogenicity. (I) 0802 - This variant has moderate previous evidence of pathogenicity in unrelated individuals. This variant has been observed in at least four heterozygous individuals with mitochondrial disease or progressive external ophthalmoplegia with ptosis (ClinVar, PMID: 28812649, PMID: 34732400, PMID: 20479361, PMID: 26469001). (SP) 0905 - No published segregation evidence has been identified for this variant. (I) 1007 - No published functional evidence has been identified for this variant. (I) 1207 - Parental origin of the variant is unresolved. Testing of this individual’s mother has shown that the variant was not maternally inherited. (I) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign -
