Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces glycine with cysteine at codon 1145 of the PALB2 protein. Computational prediction tool suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been performed for this variant. This variant has been detected in a breast cancer case-control meta-analysis in 1/60466 cases and 1/53461 unaffected individuals (PMID: 33471991; Leiden Open Variation Database DB-ID PALB2_010747). A different variant resulting in the same protein change has been reported in a BRCA1/2-negative breast cancer case-control study in 1/5589 breast cancer cases and absent in 2189 unaffected female controls (PMID: 29522266). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The c.3432_3433delCGinsAT variant (also known as p.G1145C), located in coding exon 13 of the PALB2 gene, results from an in-frame deletion of CG and insertion of AT at nucleotide positions 3432 to 3433. This results in the substitution of the glycine residue for a cysteine residue at codon 1145, an amino acid with highly dissimilar properties. This variant was reported in 1/60,466 breast cancer cases and in 1/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Based on the available evidence, the clinical significance of this variant remains unclear. -
Familial cancer of breast Uncertain:1
This sequence change replaces glycine, which is neutral and non-polar, with cysteine, which is neutral and slightly polar, at codon 1145 of the PALB2 protein (p.Gly1145Cys). Information on the frequency of this variant in the gnomAD database is not available, as this variant may be reported differently in the database. This variant has not been reported in the literature in individuals affected with PALB2-related conditions. ClinVar contains an entry for this variant (Variation ID: 484242). Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
