Variant summary
Our verdict is Uncertain significance. The variant received 2 ACMG points: 2P and 0B. PM2
The NM_024675.4(PALB2):c.1293T>G(p.Ser431Arg) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. S431N) has been classified as Likely benign.

not specified Uncertain:1
DNA sequence analysis of the PALB2 gene demonstrated a sequence change, c.1293T>G, in exon 4 that results in an amino acid change, p.Ser431Arg. This sequence change is absent from known population databases (gnomAD). The p.Ser431Arg change affects a highly conserved amino acid residue located in a domain of the PALB2 protein that is not known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Ser431Arg substitution. This sequence change does not appear to have been previously described in patients with PALB2-related disorders. Due to the lack of sufficient evidences, the clinical significance of the p.Ser431Arg change remains unknown at this time. -
Familial cancer of breast Uncertain:1
This variant is not present in population databases (ExAC no frequency). This sequence change replaces serine with arginine at codon 431 of the PALB2 protein (p.Ser431Arg). The serine residue is highly conserved and there is a moderate physicochemical difference between serine and arginine. This variant has not been reported in the literature in individuals with PALB2-related disease. ClinVar contains an entry for this variant (Variation ID:  460887). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.S431R variant (also known as c.1293T>G), located in coding exon 4 of the PALB2 gene, results from a T to G substitution at nucleotide position 1293. The serine at codon 431 is replaced by arginine, an amino acid with dissimilar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
