Genes affected
ERCC4 (HGNC:3436): (ERCC excision repair 4, endonuclease catalytic subunit) The protein encoded by this gene forms a complex with ERCC1 and is involved in the 5' incision made during nucleotide excision repair. This complex is a structure specific DNA repair endonuclease that interacts with EME1. Defects in this gene are a cause of xeroderma pigmentosum complementation group F (XP-F), or xeroderma pigmentosum VI (XP6).[provided by RefSeq, Mar 2009]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 12 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 16-13935662-AC-A is Pathogenic according to our data. Variant chr16-13935662-AC-A is described in ClinVar as [Pathogenic]. Clinvar id is 541251.Status of the report is criteria_provided_single_submitter, 1 stars.

Pathogenic, criteria provided, single submitterclinical testingInvitaeMar 14, 2018For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in ERCC4 are known to be pathogenic (PMID: 23623386). A different variant (c.1730_1731insA) giving rise to the same protein effect observed here (p.Tyr577*) has been reported in an individual affected with Cockayne syndrome (PMID: 23623389). This variant has not been reported in the literature in individuals with ERCC4-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Tyr577*) in the ERCC4 gene. It is expected to result in an absent or disrupted protein product. -
