This sequence change affects an acceptor splice site in intron 36 of the NF1 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in NF1 are known to be pathogenic (PMID: 10712197, 23913538). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with neurofibromatosis type 1 (PMID: 10980545, 18546366, 25325900, 26056819; internal data). ClinVar contains an entry for this variant (Variation ID: 547657). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. -

The NF1 c.5269-2A>G variant is predicted to disrupt the AG splice acceptor site and interfere with normal splicing. This variant is also referred to as c.5206-2A>G in an alternate transcript (NM_000267.3). This variant has been reported in multiple individuals with neurofibromatosis type 1 (see for example - Girodon-Boulandet et al. 2000. PubMed ID: 10980545; Table S3, Zhang et al. 2015. PubMed ID: 26056819). Splicing studies found this variant results in skipping of the exon which is predicted to result in a frameshift and premature protein truncation (Table S1, Zhang et al. 2015. PubMed ID: 26056819). This variant has not been reported in a large population database, indicating this variant is rare. In ClinVar, this variant has been interpreted as pathogenic (https://www.ncbi.nlm.nih.gov/clinvar/variation/547657/).
