Genes affected
CTNS (HGNC:2518): (cystinosin, lysosomal cystine transporter) This gene encodes a seven-transmembrane domain protein that functions to transport cystine out of lysosomes. Its activity is driven by the H+ electrochemical gradient of the lysosomal membrane. Mutations in this gene cause cystinosis, a lysosomal storage disorder. Alternative splicing results in multiple transcript variants. [provided by RefSeq, Jul 2009]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 12 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, LoF is a know mechanism of disease, Cryptic splice site detected, with MaxEntScore 4.7, offset of 25, new splice context is: tggGTaggg. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 17-3648932-G-A is Pathogenic according to our data. Variant chr17-3648932-G-A is described in ClinVar as Pathogenic. ClinVar VariationId is 557204.Status of the report is criteria_provided_single_submitter, 1 stars.

Studies have shown that disruption of this splice site results in skipping of exons 4 and/or 5 and introduces a premature termination codon (PMID: 12442267). The resulting mRNA is expected to undergo nonsense-mediated decay. This sequence change affects a donor splice site in intron 5 of the CTNS gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with cystinosis (PMID: 12442267). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 557204). For these reasons, this variant has been classified as Pathogenic. -
