RTEL1 (HGNC:15888): (regulator of telomere elongation helicase 1) This gene encodes a DNA helicase which functions in the stability, protection and elongation of telomeres and interacts with proteins in the shelterin complex known to protect telomeres during DNA replication. Mutations in this gene have been associated with dyskeratosis congenita and Hoyerall-Hreidarsson syndrome. Read-through transcription of this gene into the neighboring downstream gene, which encodes tumor necrosis factor receptor superfamily, member 6b, generates a non-coding transcript. Several computational tools predict a significant impact on normal splicing: Two predict the variant abolishes a canonical 5' splicing donor site and two predict the variant weakens the 5' donor site. A publication reports experimental evidence that this variant indeed affects mRNA splicing, affecting the 5' donor site and resulting in the inclusion of intron 28 which leads to a frameshift and premature termination codon; however, the affected transcript was found to have similar expression to the WT, suggesting only a portion of transcripts are impacted (Speckmann_2017). The variant was absent in 244174 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2724+5G>A has been reported in the literature as a compound heterozygous genotype in an individual affected with Dyskeratosis Congenita (Hoyeraal Hreidarsson Syndrome) (Speckmann_2017). This report does not provide unequivocal conclusions about association of the variant with Dyskeratosis Congenita (Hoyeraal Hreidarsson Syndrome). The following publication has been ascertained in the context of this evaluation (PMID: 28507545). Three submitters have cited clinical-significance assessments for this variant to ClinVar after 2014 and all classified the variant as uncertain significance.
