Variant summary
Our verdict is Likely benign. The variant received -5 ACMG points: 0P and 5B. BP4_StrongBS1_Supporting
The NM_015192.4(PLCB1):c.3590C>T(p.Thr1197Ile) variant causes a missense change. The variant allele was found at a frequency of 0.0000149 in 1,614,126 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 14/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T1197A) has been classified as Uncertain significance.

The p.T1197I variant (also known as c.3590C>T), located in coding exon 32 of the PLCB1 gene, results from a C to T substitution at nucleotide position 3590. The threonine at codon 1197 is replaced by isoleucine, an amino acid with similar properties. This variant was previously reported in the SNPDatabase as rs190019296. Based on data from the 1000 Genomes Project, the T allele has an overall frequency of approximately 0% (0/2098) total alleles studied.. Based on data from the NHLBI Exome Sequencing Project (ESP), the T allele has an overall frequency of approximately 0.01% (1/13006) total alleles studied, having been observed in 0.02% (1/4406) African American alleles. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear. -

Developmental and epileptic encephalopathy, 12 Uncertain:1
This sequence change replaces threonine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 1197 of the PLCB1 protein (p.Thr1197Ile). This variant is present in population databases (rs190019296, gnomAD 0.04%). This variant has not been reported in the literature in individuals affected with PLCB1-related conditions. ClinVar contains an entry for this variant (Variation ID: 471509). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
