Genes affected
DKC1 (HGNC:2890): (dyskerin pseudouridine synthase 1) This gene functions in two distinct complexes. It plays an active role in telomerase stabilization and maintenance, as well as recognition of snoRNAs containing H/ACA sequences which provides stability during biogenesis and assembly into H/ACA small nucleolar RNA ribonucleoproteins (snoRNPs). This gene is highly conserved and widely expressed, and may play additional roles in nucleo-cytoplasmic shuttling, DNA damage response, and cell adhesion. Mutations have been associated with X-linked dyskeratosis congenita. Alternative splicing results in multiple transcript variants. [provided by RefSeq, Jan 2014]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PM1
In a hotspot region, there are 2 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 5 uncertain in NM_001363.5
PM2
Very rare variant in population databases, with high coverage;
PP2
Missense variant where missense usually causes diseases, DKC1
PP3
MetaRNN computational evidence supports a deleterious effect, 0.906
PP5
Variant X-154774669-C-T is Pathogenic according to our data. Variant chrX-154774669-C-T is described in ClinVar as [Conflicting_classifications_of_pathogenicity]. Clinvar id is 38941.We mark this variant Likely_pathogenic, oryginal submissions are: {Likely_pathogenic=2, not_provided=1, Uncertain_significance=1}. Variant chrX-154774669-C-T is described in Lovd as [Pathogenic].

Dyskeratosis congenita Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingInvitaeJan 19, 2020In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has been observed in individuals affected with dyskeratosis congenita (PMID: 16332973, Invitae). ClinVar contains an entry for this variant (Variation ID: 38941). This variant is not present in population databases (ExAC no frequency). This sequence change replaces threonine with isoleucine at codon 408 of the DKC1 protein (p.Thr408Ile). The threonine residue is highly conserved and there is a moderate physicochemical difference between threonine and isoleucine. -
