This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 1075 of the KCNH2 protein (p.Pro1075Leu). This variant is present in population databases (rs199473028, gnomAD 0.01%). This missense change has been observed in individuals with long QT syndrome (PMID: 17905336, 20386770, 20850565). ClinVar contains an entry for this variant (Variation ID: 67481). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on KCNH2 function (PMID: 31557540, 34930020). The c.3224C>T (p.P1075L) alteration is located in exon 14 (coding exon 14) of the KCNH2 gene. This alteration results from a C to T substitution at nucleotide position 3224, causing the proline (P) at amino acid position 1075 to be replaced by a leucine (L). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.

This missense variant replaces proline with leucine at codon 1075 of the KCNH2 protein. This variant is found within a highly conserved region in the C-terminal cytoplasmic domain (a.a. 843-1159). Rare non-truncating variants in this region have been shown to be significantly overrepresented in individuals with long QT syndrome (PMID: 32893267). Functional studies have provided conflicting observations regarding the impact of this variant on KCNH2 channel function (PMID: 31557540, 34930020). This variant has been reported in two individuals affected with long QT syndrome (PMID: 17905336, 20850565), in an individual affected with complete atrioventricular block and torsades de pointes (PMID: 20386770), in an individual suspected of having epilepsy (PMID: 31696929). This variant has also been identified in three individuals without arrhythmia (PMID: 34930020), as well as in 5/249490 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
