SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. p.Thr1645Met (T1645M) ACG>ATG: c.4934 C>T in exon 28 of the SCN5A gene (NM_198056.2) The T1645M mutation in the SCN5A gene has been reported in one 15 year-old individual who presented with a marginally prolonged QTc (0.450 sec) by resting EKG, but prolonged QTc (0.551 sec) by Holter (Wattanasirichaigoon D et al., 1999). This individual's sister had a history of seizures and syncope in childhood, and died suddenly at the age of 18. The father and paternal aunt did not have prolonged QTc intervals, but required pacemakers for bradycardia and syncope (Wattanasirichaigoon D et al., 1999). None of these family members underwent genetic testing. T1645M is a non-conservative amino acid substitution as these residues differ in polarity, charge, size and/or other properties and is more likely to impact secondary structure. The T1645 residue is highly conserved across species. Mutations in nearby residues (R1644C, R1644H, A1649V, L1650F) have been reported in association with arrhythmia, further supporting the functional importance of this region of the protein. Additionally, the T1645M mutation was not observed in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The variant is found in CARDIOMYOPATHY panel(s). -
