SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. The S1787T variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. Furthermore, missense mutations in nearby residues (E1784K, L1786Q, D1790G) have been reported in association with arrhythmia, supporting the functional importance of this region of the protein. However, the S1787T variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. Finally, a different missense substitution in the same residue (S1787N) has been reported both in association with LQTS and as a polymorphism in healthy control populations (Splawski I et al., 2000; Ackerman M et al., 2004; Kapplinger J et al., 2010).Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in DCM-CRDM panel(s). -
