SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. Alternative splicing results in several transcript variants encoding different isoforms. [provided by RefSeq, May 2022]

This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with atrial fibrillation (PMID: 18929244). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 68002). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects SCN5A function (PMID: 18929244). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
Likely pathogenic, criteria provided, single submitterclinical testingGeneDxJun 27, 2016The Met1875Thr mutation in the SCN5A gene has been reported in association with atrial fibrillation in multiple individuals from three generations of a Japanese family (Makiyama T et al., 2008). Six individuals had frequent premature atrial contractions (PACs) or atrial fibrillation, and only one individual had a prolonged QTc interval. Met1875Thr was absent from 210 Japanese control individuals in this study (Makiyama T et al., 2008). In addition, Met1875Thr was not observed in approximately 6,300 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Met1875Thr results in a non-conservative amino acid substitution of a non-polar Methionine with a polar Threonine at a position that is highly conserved across species (Makiyama T et al., 2008). Mutations in nearby residues (Val1861Ile, Lys1872Asn, Arg1897Trp) have been reported in association with arrhythmia, further supporting the functional importance of this region of the protein. Functional studies showed a pronounced depolarized shift in steady-state inactivation and no persistent inward sodium currents, which is consistent with a unique gainof- function type of modulation (Makiyama T et al., 2008). -
