Variant summary
Our verdict is Likely pathogenic. The variant received 8 ACMG points: 8P and 0B. PM1PM2PP3_ModeratePP5_Moderate
The NM_001099404.2(SCN5A):c.4507T>C(p.Ser1503Pro) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 14/24 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Synonymous variant affecting the same amino acid position (i.e. S1503S) has been classified as Benign.

Cardiovascular phenotype Pathogenic:1
The p.S1503P variant (also known as c.4507T>C), located in coding exon 25 of the SCN5A gene, results from a T to C substitution at nucleotide position 4507. The serine at codon 1503 is replaced by proline, an amino acid with similar properties, and is located in the DIII/DIV interdomain linker region. This variant has been determined to be the result of a de novo mutation or germline mosaicism in one individual with prolonged QT syndrome (Ambry internal data). This amino acid position is highly conserved in available vertebrate species. Based on internal structural analysis, this variant is more disruptive than known pathogenic variants (Shen H et al. Science, 2017 03;355:[Epub ahead of print]). In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
