Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 6.4e-05 in 251068 control chromosomes (gnomAD). This frequency is not higher than expected for a pathogenic variant in RAG1 causing Severe Combined Immunodeficiency Syndrome/Omenn Syndrome (6.4e-05 vs 0.00071), allowing no conclusion about variant significance. c.1421G>A has been reported in the literature in multiple individuals affected with Severe Combined Immunodeficiency Syndrome/Omenn Syndrome (e.g. Corneo_2001, Villa_2001, Aluri_2019, Farmer_2019). These data indicate that the variant is very likely to be associated with disease. Experimental evidence evaluating an impact on protein function demonstrated the variant to have decreased V(D)J recombination activity in vitro (Corneo_2001). Experimental studies have shown that this missense change affects RAG1 function (PMID: 11313270). This variant disrupts the p.Arg474 amino acid residue in RAG1. Other variant(s) that disrupt this residue have been observed in individuals with RAG1-related conditions (19064334and16960852), which suggests that this may be a clinically significant amino acid residue.
