Variant summary
Our verdict is Pathogenic. The variant received 19 ACMG points: 19P and 0B. PM1PM2PM5PP2PP3_StrongPP5_Very_Strong
The NM_000487.6(ARSA):c.917C>T(p.Thr306Met) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,460,934 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T306A) has been classified as Likely pathogenic.

A heterozygous missense variant in exon 5 of the ARSA gene that results in the amino acid substitution of Methionine for Threonine at codon 306 was detected. The observed variant c.917C>T (p.Thr306Met) has not been reported in the 1000 genomes and gnomAD databases. The in silico prediction of the variant is damaging by MutationTaster2, SIFT and PolyPhen2. The reference codon is conserved across species. In summary, the variant meets our criteria to be classified as pathogenic. -

This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 306 of the ARSA protein (p.Thr306Met). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with metachromatic leukodystrophy (PMID: 18786133, 30057904; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 68159). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ARSA protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects ARSA function (PMID: 19606494). For these reasons, this variant has been classified as Pathogenic. -
Variant summary: ARSA c.917C>T (p.Thr306Met) results in a non-conservative amino acid change located in the Sulfatase, N-terminal domain (IPR000917) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 247446 control chromosomes (gnomAD). c.917C>T has been reported in the literature in individuals affected with Metachromatic Leukodystrophy (e.g. Biffi_2008, Chen_2018, Li_2018). These data indicate that the variant is likely to be associated with disease. Experimental evidence evaluating an impact on protein function demonstrated null residual enzyme activity for the variant in different cellular systems (Cesani_2009). Two ClinVar submitters (evaluation after 2014) cite the variant as pathogenic (n=1) and as uncertain significance (n=1). Based on the evidence outlined above, the variant was classified as pathogenic. -

Published functional studies demonstrate a damaging effect as T306M caused a loss of enzyme activity (PMID: 19606494); Not observed at significant frequency in large population cohorts (gnomAD); Identified in a patient with metachromatic leukodystrophy in published literature (PMID: 18786133); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Also known as p.T304M; This variant is associated with the following publications: (PMID: 34554397, 31069529, 26915897, 19606494, 33855715, 30057904, 18786133) -
