SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. The arginine at codon 1739 is replaced by glutamine, an amino acid with highly similar properties, and is located in the DIV-S5/S6T transmembrane-spanning region. This variant has been detected in an individual reported to have dilated cardiomyopathy (Maekawa K et al. Ann. Hum. Genet., 2005 Jul;69:413-28). This alteration has also been reported as a secondary cardiac variant in an exome cohort, and has been identified in a hospital-based cohort in an individual not indicated as having a related cardiovascular phenotype; however, details were limited (Ng D et al. Circ Cardiovasc Genet, 2013 Aug;6:337-46; Van Driest SL et al. JAMA. 2016 Jan;315(1):47-57). This amino acid position is not well conserved in available vertebrate species.
