Genes affected
LRBA (HGNC:1742): (LPS responsive beige-like anchor protein) The protein encoded by this gene is a member of the WDL-BEACH-WD (WBW) gene family. Its expression is induced in B cells and macrophages by bacterial lipopolysaccharides (LPS). The encoded protein associates with protein kinase A and may be involved in leading intracellular vesicles to activated receptor complexes, which aids in the secretion and/or membrane deposition of immune effector molecules. Defects in this gene are associated with the disorder common variable immunodeficiency-8 with autoimmunity. Two transcript variants encoding different isoforms have been found for this gene. ACMG classification

Classification made for transcript

Verdict is Likely_benign. Variant got -3 ACMG points.

PM2
Very rare variant in population databases, with high coverage;
BP4
Computational evidence support a benign effect (MetaRNN=0.03675583).
BS1
Variant frequency is greater than expected in population eas. gnomad4_exome allele frequency = 0.0000513 (75/1461832) while in subpopulation EAS AF= 0.00134 (53/39698). AF 95% confidence interval is 0.00105. There are 0 homozygotes in gnomad4_exome. There are 35 alleles in male gnomad4_exome subpopulation. Median coverage is 32. This position pass quality control queck. Existence of Clinvar submissions makes me limit the strength of this signal to Supporting

Combined immunodeficiency due to LRBA deficiency Uncertain:2
Uncertain significance, criteria provided, single submitterclinical testingInvitaeNov 28, 2023This sequence change replaces valine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 1695 of the LRBA protein (p.Val1695Ala). This variant is present in population databases (rs200935054, gnomAD 0.1%). This missense change has been observed in individual(s) with common variable immunodeficiency (PMID: 30363934). ClinVar contains an entry for this variant (Variation ID: 473179). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt LRBA protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
