APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. The methionine at codon 18 is replaced by lysine, an amino acid with similar properties. This alteration has been observed in healthy controls in multiple studies, including in 1/2512 control individuals from a healthy population database, 1/490 cancer-free elderly controls in a Turkish population, in a cohort of 572 atherosclerosis patients with no clinical history of cancer, and as a secondary finding in 1 out of 572 ClinSeq participants, unselected for personal or family history of cancer, who underwent exome sequencing (Rosenthal EA et al. Hum Genet, 2018 Oct;137:795-806; Akcay IM et al. Int J Cancer, 2021 01;148:285-295; Pinard A et al. Hum Mutat, 2016 12;37:1299-1307; Johnston JJ et al. Am J Hum Genet, 2012 Jul;91:97-108). In addition, this alteration was determined to retain normal APC cofactor activity in one experimental study (He X et al. J Biol Chem, 1998 Oct;273:27449-58). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
