Variant summary: CBS c.1411G>A (p.Gly471Arg) results in a non-conservative amino acid change located in the CBS domain (IPR000644) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8.8e-05 in 251400 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in CBS causing Homocystinuria (8.8e-05 vs 0.003), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.1411G>A in individuals affected with Homocystinuria and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 212882). Based on the evidence outlined above, the variant was classified as uncertain significance. -

This variant is present in population databases (rs201098477, gnomAD 0.04%). This variant has not been reported in the literature in individuals affected with CBS-related conditions. ClinVar contains an entry for this variant (Variation ID: 212882). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The G471R variant has not been published as pathogenic in association with homocystinuria or been reported as benign to our knowledge. The G471R variant is observed in 5/11554 (0.043%) alleles from individuals of Latino background in the Exome Aggregation Consortium (ExAC) dataset, though no individuals were reported to be homozygous (Lek et al., 2016). The G471R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. -
