The previously defined group FANCH is the same as FANCA. Fanconi anemia is a genetically heterogeneous recessive disorder characterized by cytogenetic instability, hypersensitivity to DNA crosslinking agents, increased chromosomal breakage, and defective DNA repair. The members of the Fanconi anemia complementation group do not share sequence similarity; they are related by their assembly into a common nuclear protein complex. This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -

The FANCC c.1544C>G (p.Thr515Ser) missense change has a maximum subpopulation frequency of 0.0070% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). The in silico tool REVEL predicts a benign effect on protein function, but this prediction has not been confirmed by functional studies. In a case-control study, this variant was reported in 1 of 1441 individuals with breast cancer and absent in 4643 control individuals (PMID: 23028338). This variant has not been reported in the literature in individuals with Fanconi anemia. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

This sequence change does not appear to have been previously described in patients with FANCC-related disorders and has been described in the gnomAD database with a low population frequency of 0.0043% (dbSNP rs201379302). The p.Thr515Ser change affects a highly conserved amino acid residue located in a domain of the FANCC protein that is known to be functional. The p.Thr515Ser substitution appears to be benign using several in-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL). Due to this insufficient evidence and the lack of functional studies, the clinical significance of the p.Thr515Ser change remains unknown at this time. -

This sequence change replaces threonine, which is neutral and polar, with serine, which is neutral and polar, at codon 515 of the FANCC protein (p.Thr515Ser). This variant is present in population databases (rs201379302, gnomAD 0.008%). This variant has not been reported in the literature in individuals affected with FANCC-related conditions. ClinVar contains an entry for this variant (Variation ID: 367609). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The serine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. The p.T515S variant (also known as c.1544C>G), located in coding exon 14 of the FANCC gene, results from a C to G substitution at nucleotide position 1544. The threonine at codon 515 is replaced by serine, an amino acid with similar properties. In one study, this alteration was detected in 1/1441 breast cancer cases and 0/464 healthy controls (Thompson ER et al. PLoS Genet, 2012 Sep;8:e1002894). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
