This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 246 of the HMBS protein (p.Arg246His). This variant is present in population databases (rs201909197, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with HMBS-related conditions. ClinVar contains an entry for this variant (Variation ID: 452525). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt HMBS protein function with a negative predictive value of 80%. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on HMBS function (PMID: 27539938, 29360981).
