Genes affected
TG (HGNC:11764): (thyroglobulin) Thyroglobulin (Tg) is a glycoprotein homodimer produced predominantly by the thryroid gland. It acts as a substrate for the synthesis of thyroxine and triiodothyronine as well as the storage of the inactive forms of thyroid hormone and iodine. Thyroglobulin is secreted from the endoplasmic reticulum to its site of iodination, and subsequent thyroxine biosynthesis, in the follicular lumen. Mutations in this gene cause thyroid dyshormonogenesis, manifested as goiter, and are associated with moderate to severe congenital hypothyroidism. Polymorphisms in this gene are associated with susceptibility to autoimmune thyroid diseases (AITD) such as Graves disease and Hashimoto thryoiditis. [provided by RefSeq, Nov 2009]

Pathogenic, criteria provided, single submitterclinical testingGeneDxOct 11, 2019Published functional studies demonstrate a damaging effect; insertion of homolgus mutation into mouse AChE cause intracellular retention of the soluble and the GPI-anchored protein and impair AChE function (Raef et al., 2010); In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; Also known as R2223H using alternate nomenclature; This variant is associated with the following publications: (PMID: 30542390, 19438905, 12915634, 20089614, 23035660, 29546359) -
Pathogenic, criteria provided, single submitterclinical testingInvitaeJun 28, 2023For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects TG function (PMID: 23035660). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TG protein function. ClinVar contains an entry for this variant (Variation ID: 12700). This variant is also known as R2223H. This missense change has been observed in individual(s) with congenital hypothyroidism (PMID: 12915634, 20089614). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This variant is present in population databases (rs2069566, gnomAD 0.003%). This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 2242 of the TG protein (p.Arg2242His). -
