Variant summary

Our verdict is Pathogenic. The variant received 25 ACMG points: 25P and 0B. PS1PS3PM1PM2PP2PP3_StrongPP5_Very_Strong

The NM_001184880.2(PCDH19):c.1671C>G(p.Asn557Lys) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV002557271: "This variant has strong functional evidence supporting abnormal protein function. Functional studies show that this variant causes reduced transcriptional activity and premature differentiation of human neural stem and progenitor cells." PMID:28334947, 29763708". Another nucleotide change resulting in the same amino acid substitution has been previously reported as Pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N557S) has been classified as Uncertain significance.

Genes affected
PCDH19 (HGNC:14270): (protocadherin 19) The protein encoded by this gene is a member of the delta-2 protocadherin subclass of the cadherin superfamily. The encoded protein is thought to be a calcium-dependent cell-adhesion protein that is primarily expressed in the brain. Mutations in this gene on human chromosome X are associated with sporadic infantile epileptic encephalopathy and to a female-restricted form of epilepsy (EFMR; also known as PCDH19RE). Multiple transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jul 2017]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 25 ACMG points.

PS1
Transcript NM_001184880.2 (PCDH19) is affected with MISSENSE_VARIANT having same AA change as one Pathogenic present in ClinVar.
PS3
PS3 evidence extracted from ClinVar submissions: SCV002557271: "This variant has strong functional evidence supporting abnormal protein function. Functional studies show that this variant causes reduced transcriptional activity and premature differentiation of human neural stem and progenitor cells." PMID:28334947, 29763708
PM1
In a hotspot region, there are 6 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 11 uncertain in NM_001184880.2
PM2
Very rare variant in population databases, with high coverage;
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 107 curated pathogenic missense variants (we use a threshold of 10). The gene has 20 curated benign missense variants. Gene score misZ: 2.5929 (below the threshold of 3.09). Trascript score misZ: NaN (below the threshold of 3.09). GenCC associations: The gene is linked to developmental and epileptic encephalopathy, 9, X-linked complex neurodevelopmental disorder, Dravet syndrome.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.979
PP5
Variant X-100406927-G-C is Pathogenic according to our data. Variant chrX-100406927-G-C is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 11022.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
